Status of Carbapenemase producing Enterobacteriaceae in Korea, 2014

Abstract
Background: Year after year, the prevalence of antimicrobial resistance bacteria has increased due to corresponding increase in usage of broad spectrum antibiotics. Particularly, Carbapenemase producing Enterobactericeae (CPE) has become an important global health concern, since there has been an increase in the ratio of some antimicrobial resistant bacteria like Extended-spectrum beta-lactamases (ESBL) that use carbapenem. In this paper, we describe the occurrence of CPE in Korea, baseline characteristics of the patients who had CPE colonies, and the ratio of Carbapenemase.

Methods: From January to December 2014, we analyzed a total of 174 reported CPE cases in Korea through sample monitoring surveillance system. Data collection was based on epidemiologic survey, and statistical analysis was conducted using the Excel program.

Results: Rectal swabbing of the specimens confirmed the highest number of patients (52 patients, 29.9%), while 55.7% of CPE colonized patients were male. It was also determined that CPE patients had past histories of cardiovascular disease (57 patients, 32.8%), cancer (38 patients, 21.8%), and diabetes mellitus (37, 21.3%).
The risk factors of CPE colonization (e.g., admission in Intensive Care Unit, mechanical ventilation, insertion of invasive catheter, and usage of antibiotics) were present in more than half of the total number of reported cases.
Klebsiella pneumoniae is the most popular Enterobactericeae which was confirmed in CPE (104 cases, 59.8%), and OXA-232 is the most common carbapenemase in confirmed cases (48 cases, 27.6%).

Conclusion: CPE is the important bacteria that can bring about associated infections. Thus, health authorities should conduct accurate surveillance of CPE occurrence and establish adequate policies to prevent transmission of CPE.
